Support seniors and their families in exploring private senior residences and home care options. Provide personalized support based on attentive listening, understanding of needs, and trust. Facilitate transitions to a new living environment with professionalism and empathy. Develop lasting relationships with partners (healthcare professionals, community organizations, and local stakeholders). Ensure diligent follow-up and maintain organized case and file management.
