Liquid Personnel is seeking an experienced Occupational Therapist to support the delivery of adult social care through timely and effective initial assessments for its client based in London Borough of Tower Hamlets. This role offers the opportunity to work with service users in the community for up to six weeks on tailored rehabilitation programmes, supported by in-house Reablement Officers (ROs) who deliver care under your direction. The goal: prevent, reduce, or delay long-term care needs. What will your responsibilities be? Manage a varied caseload of hospital discharges and community referrals. Complete risk-assessed home visits and provide clinical oversight for reablement plans. Direct and support Reablement Officers delivering care programmes. Work collaboratively with a strong multidisciplinary team to achieve positive outcomes. Qualifications & Experience: HCPC registered Occupational Therapist. Minimum 2 years' experience post-qualification (Band 6 level or equivalent). Proven experience in case work and physical rehabilitation in a busy environment. London-based for urgent case management. Ability to work flexibly: Monday-Friday, some remote work, and office-based two days/week.
