About the role
The Registered Nurse (RN) Case Manager is responsible for overseeing and coordinating healthcare services for individuals with intellectual and developmental disabilities (IDD) in community-based and ICF settings. - Monitor incident reports and follow up on reportable health events per DDS guidelines
- Maintain accurate documentation aligned with audit and compliance standards
Requirements

- Collaboration & Communication

- Active Registered Nurse (RN) License in Washington, DC
- Minimum 2 years of nursing experience
- Strong knowledge of nursing practices and care coordination

- Ensure availability of medical supplies and resources
- Maintain current licensure documentation for all nursing personnel
- Perform other duties as assigned
- Experience in IDD, ICF, or community-based services
- Knowledge of DDS, DDA, DOH, ISP, HCMP, and IDT processes
- Familiarity with DDS, DOH, or DDA regulations
- Experience supervising or training staff
- Experience with EHR systems
- Work Environment
- Field-based role across multiple residential and program locations, with administrative work
- Local travel required (mileage reimbursement provided)
- Monday-Friday schedule with On-call rotation required
