Carilion's flagship hospital, Carilion Roanoke Memorial Hospital, is a 763-bed academic medical center, level 1 trauma center, tertiary/quaternary referral center, and Certified Primary Stroke center that has consistently been ranked by US News as a Best Hospital in the state of Virginia. It is also affiliated with the Virginia Tech Carilion School of Medicine and has numerous post-graduate training programs, including multiple medical and surgical residencies and fellowships. Clinical Staff I pharmacists are a core group of clinical pharmacists who rotate through various decentralized zones, including 5 adult patient care zones (hospitalist services, trauma, oncology, etc.), 1 pediatric zone, and 1 medication history zone in the main hospital. Zone assignments are determined based on clinical need, as well as pharmacist interests. Pharmacists will be responsible for addressing a variety of pharmacokinetic medication needs and will have autonomy through use of multiple P&T approved pharmacy protocols. Pharmacists will assist with optimizing patients' medication regimens throughout the continuum of care. Long-term opportunities may include the ability to train in other services lines (ICU, ID, IM, pediatrics, among others) to provide cross coverage. Clinical Staff I pharmacists are responsible for working every third weekend, as well as 1 major and 1 minor holiday annually. CRMH also has a pharmacy residency program comprised of 13 residents, including seven PGY1s, and six PGY2s in Internal Medicine, Critical Care, Infectious Diseases, Emergency Medicine, and Health System Pharmacy Administration and Leadership. Clinical Staff I pharmacists can assist with precepting residents within several of the decentralized zones, as well as participate in other resident learning experiences. Various other clinical experiences such as committee participation, process improvement, and research are available. Qualified candidates must be a graduate of a College of Pharmacy accredited by the Accreditation Council for Pharmacy Education (ACPE) and must possess a current Virginia pharmacist license or be eligible for licensure. An ASHP-accredited PGY1 Pharmacy Residency is preferred. Applicants must possess excellent leadership, communication, interpersonal and teamwork skills, as well as demonstrate advanced skills in critical thinking and analysis. Must be able to effectively budget time, work efficiently and accurately, and follow procedures with minimum supervision. The Pharmacist Participates as an active member of interdisciplinary care teams Advises on the most efficacious treatments and participates in pharmacotherapeutic decision making. Assists with drug information and evidence-based medicine. Consults and provides healthcare providers advice on medications. May need to respond to patient emergencies. Reviews and verifies medication orders for accuracy. Assures preparation of sterile/nonsterile compounding according to Standard Operating Procedures. Assures delivery of medications in a timely manner. Provides education and training for healthcare providers (pharmacy, nursing and medical), students, residents, patients, and patient families. Provides feedback to pharmacy students/residents. Maintains competency in all technologies utilized by the Department of Pharmacy. Supports clinical pharmacy initiatives of the institution. Maintains compliance with statutes, laws, regulations, standards and policies governing the practice of pharmacy. Facilitates age specific pharmaceutical care to patients. What We Require: Education: Graduate of a College of Pharmacy accredited by the Accreditation Council for Pharmacy Education (ACPE). Experience: Pharmacy Practice Residency or equivalent Hospital experience is preferred Licensure, certification, and/or registration: License to practice Pharmacy in the Commonwealth of Virginia required within 90 days of hire. During the first 90 days of employment and the licensure application process, pharmacists without a valid license to practice pharmacy in the Commonwealth of Virginia may not perform duties restricted to a licensed pharmacist until the license is obtained. If the license is not obtained within 90 days of employment, the employee may be subject to separation from Carilion Clinic.
