Nurse Practitioner - Advanced Heart Failure Services - Queens Medical Group As part of the Advanced Heart Failure team, the Nurse Practitioner will play a central role in both inpatient and outpatient care delivery, helping improve transitions of care and long-term outcomes for this high-risk patient population. Duties will include but not limited to the following:

Responsibilities

  • Provide comprehensive care to advanced heart failure patients in collaboration with heart failure cardiologists and the multidisciplinary team
  • Participate in both outpatient clinic care and inpatient service responsibilities
  • Conduct inpatient consultations for the Advanced Heart Failure service
  • Facilitate outpatient management, including:
  • Telephone triage of patient symptoms
  • Review and follow-up of diagnostic results and lab findings
  • Medication titration and care coordination
  • Support transitions of care between hospital discharge and outpatient follow-up
  • Participate in the evaluation and workup of patients for advanced heart failure therapies (including mechanical circulatory support and transplant evaluation as applicable)
  • Provide care for patients with advanced heart failure devices, including management of patients with remote pulmonary artery pressure monitoring systems
  • Deliver care in accordance with Practice Protocols, Clinical Practice Agreements, and institutional policies
  • This is a 4 days per week, day shift position based in Flushing, NY, with responsibilities across both inpatient and outpatient settings.
  • Preferred Criteria:
  • 2+ years of Nurse Practitioner experience preferred
  • Prior acute care RN experience in cardiology strongly preferred
  • Experience in heart failure or cardiology (inpatient and/or outpatient)
  • Required Criteria:
  • Bachelor of Science in Nursing; Master's Degree as a Nurse Practitioner.
  • Current New York State License as Nurse Practitioner.
  • Current New York State RN license.
  • Certification as a Nurse Practitioner from a New York State approved accrediting body - American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP).
  • ACLS and BCLS Certification from the American Heart Association
  • Demonstrated critical thinking and analytical skills, with the ability to assess complex clinical situations and develop appropriate care plans
  • Excellent verbal and written communication skills, with the ability to effectively interact with patients, families, and multidisciplinary team members
  • Strong team-oriented mindset, with the ability to collaborate closely with heart failure cardiologists and healthcare professionals to achieve shared patient care goals
  • Ability to manage multiple priorities in a fast-paced clinical environment while maintaining attention to detail
