Position Overview United Health Services (UHS) is seeking an experience Pharmacy Technician for their Medication History Pharmacy Technician position. Medication History Pharmacy Technician obtains a comprehensive and accurate medication history within 24 hours of admission through direct interviews with the patient and/or caregiver, as well as communication with the patient's provider and outpatient pharmacy. Primary Department, Division, or Unit: Pharmacy Services, UHS Wilson Medical Center Primary Work Shift: Rotational Regular Scheduled Weekly Hours: 40 Compensation Range: $22.97 - $34.46 per hour, depending on experience ----- Job Responsibilities: Interview newly admitted patients and/or caregivers to obtain a complete and accurate home medication history, including prescription, over-the-counter, and herbal products. Collect and verify patient allergy information, ensuring completeness and accuracy. Utilize available electronic health records and pharmacy databases to validate medication histories and identify discrepancies. Accurately document medication and allergy information in electronic medical records or designated paper forms in accordance with organizational policies. Communicate pertinent medication-related information, discrepancies, or concerns to pharmacists, physicians, and other healthcare professionals in a timely manner. Collaborate with the healthcare team to support medication reconciliation and promote patient safety.
