What will your responsibilities be? Carry out needs assessments: Undertake Care Act assessments to identify the support requirements of adults with learning disabilities. Develop and review care plans: Work collaboratively with individuals, families, and multi-disciplinary professionals to create tailored support packages that promote independence and well-being.
