FTE: 1_, Shift: day_, Schedule: M-F _ The Bed Readiness Case Manager is a hybrid role based in either the Spokane or Yakima market, with primary responsibility for supporting both Inland Northwest (INW) and Yakima regions. This position conducts on-site rounding within the local hospital and with contracted post-acute partners (e.g., SNF, ALF, AFH) to facilitate timely transitions of care and maintain strong community relationships. While the role is physically based in one market, it also provides remote oversight and coordination for the second market to ensure consistent referral management, patient flow, and placement outcomes across both regions. Position Summary The Bed Readiness Case Manager (BRCM) for Post-Acute Care (PAC) provides exemplary cross-continuum care coordination, working with physicians, internal teams, and community partners to manage and optimize patient care. This role identifies opportunities to improve care delivery, advance population health, and strengthen collaboration, including increasing utilization of designated MultiCare post-acute services, expanding into new markets, and growing market share. As a highly visible professional, the BRCM maintains extensive engagement with the post-acute care community and develops a deep understanding of community-based service needs and dynamics. Responsibilities: Manage Length of Stay (LOS) and Rehospitalization Risk Coordinate Cross-Continuum Care Drive Bed Readiness and Network Utilization Manage Community Account Relationships Identify Performance Improvement Opportunities Support Market Growth and Expansion Act as a Subject Matter Expert (SME) Lead Communication and Escalation Pathways Excellent communication and relationship management skills, with the ability to engage providers, internal stakeholders, and leadership effectively. Proficiency in Microsoft Office tools (Excel, Word, Outlook, Teams) and ability to work with data reporting tools (e.g., Power BI) to support performance monitoring.
