Social Worker - Behavioral Health Unit - Full Time

About the role

Within Southern NH Medical Centers Behavioral Health Unit we understand the important connection between physical, mental, and emotional well-being and have the resources to support our community through inpatient and outpatient behavioral health services, from emergency care, to intensive outpatient programs. This 18-bed voluntary unit serves as an essential healing process within our community. The Social Worker assesses and evaluates the patient's health status and the patient's/family's significant other's response to illness and provides supportive counseling and community resource referrals. They collaborate with the care team to educate patients and their families on next steps.

Responsibilities

• Conduct patient assessments, utilization reviews
• Provide resources and community referrals for patient support services
• Participate in discharge rounds and facilitate case conferences
• Care coordination with families, providers, and the clinical team
• Facilitate staff de-briefing meetings

Requirements

• Master's Degree in Social Work required
• Minimum of two years acute care social work experience preferred.
• Professional certification in social work and LICSW licensure or additional certifications are desirable but not required.
