HOW YOU'LL WORK:MAJOR AREAS OF RESPONSIBILITY:Plan of Care: Provide skilled physical therapy as ordered by the attending physician. Observe, document, and report patient responses, progress, and any changes in condition. Conduct OASIS assessments to establish and adjust individualized plans of care as needed.Patient Outcomes: Deliver interventions to relieve pain, restore or improve function, and maintain optimal performance using therapeutic techniques such as exercise, manual therapy, and modalities.Minimize Patient Risk: Reduce re-hospitalizations by prioritizing visits for high-risk patients, providing education on preventive measures, and promoting informed decision-making.Family Educator/Advocate: Educate patients and families on disease or injury management throughout all phases of therapy. Provide instruction on the use of mobility aids and adaptive equipment, including wheelchairs, braces, walkers, crutches, canes, and prosthetic or orthotic devices.Supervision: Supervise and oversee therapy services provided by Physical Therapist Assistants (PTAs) and, when appropriate, nurse aides.Communication/Collaboration: Maintain consistent communication with PTAs and nurse aides regarding supervisory visit findings and care coordination.Policies: Complete all clinical documentation in accordance with agency protocols and Medicare/Federal guidelines.Rules and Regulations: Adhere to all agency policies, procedures, and regulatory requirements. Communicate any changes in schedule or availability to appropriate team members.Operations: Participate in required in-service training and attend mandatory organizational meetings.HARD AND SOFT SKILLS:Strong communication skills with a compassionate and positive approachPatience and professionalism when working with patients, families, physicians, and colleaguesAttention to detail and the ability to observe, follow directions, and ensure accurate documentationHealth insuranceDental insuranceVision insurance401(k)Flexible schedulePerformance bonus
