  • Coordinates family conferences with clinical care team based on clinical changes/milestone in case.

  • Delegates and supervises care in accordance with nurse practice acts and professional nursing role.
  • Initiates multidisciplinary collaboration to positively impact the outcomes of health provided.
  • Initiates steps to support and enhance the patient's responsibility and self-determination in decision-making concerning health, treatment and well-being.
  • Involves the individual patient/family in identification of requirements/perceptions of learning needs and preparation to assume self-care on discharge.
  • Shares knowledge and acts as a professional resource to other members of the health care team.
  • Supports and participates in Unit Based Council initiatives.
  • Contributes to professional growth of peers by providing feedback through peer coaching and evaluation.

  • Assesses, utilizing age specific data, the patient's physical, psychosocial, and spiritual status in a comprehensive manner.
  • Demonstrates accurate assessment skills, appropriate to the age of the patient, based on scientific facts and principles.
  • Performs initial assessment and completes required documentation within organizational standards.
  • Performs and documents ongoing assessments including observation, physical examination, laboratory/test results, and patient response to procedural intervention; reviews completed charts daily and places any missing order as needed.
  • Participates in the identification and clarification of patient needs, as evidenced by participation in multidisciplinary care planning, to achieve optimal patient outcomes.
  • Reviews and revises plan of care and treatment plan according to patient's status.
  • Completes patient classification for assigned patients as required.
  • Incorporates patient teaching/discharge planning to promote optimal patient outcomes.
  • Provides, evaluates retention of, and documents education to the patient/family/significant other according to organizational standards.
  • Coordinates and documents appropriate discharge planning and referrals to ensure continuity of care after discharge.
  • Provides pre-operative and post-operative teaching on all new surgical patient, if applicable.
  • Sees patients independently under Nurse Visit Only, those on infusion/active treatments, for pre-op surgical consent review, for post-op voiding trials, wound care and staple removal, if applicable.

Collaborates in the development, execution and evaluation of the multidisciplinary plan of care. Establishes goals and strategies for meeting the care coordination, discharge, and/or continuing care needs across settings with the patient, family, and/or other care providers. Job Description: Education: Required: Bachelor of Science in Nursing (BSN) or Master's degree in Nursing Licensure/Certification: Required: California RN licensure; Oncology Certified Nurse within three (3) years of hire if assigned to Hematology Oncology department (CC7651); American Heart Association BLS
