Liquid Personnel is recruiting an Adult Social Worker to join its client's CHC team, based in Somerset. This is a key opportunity to contribute to high-quality Continuing Healthcare (CHC) assessments and support robust, evidence-based decision making within a multi-disciplinary environment. What will your responsibilities be? Complete comprehensive CHC assessments across a range of care settings in line with the National Framework. Lead and contribute to Decision Support Tool (DST) processes with accuracy and sound professional judgement. Act as a core member of the multi-disciplinary team, representing the local authority perspective. Provide constructive challenge to ensure consistent and lawful CHC eligibility decisions. Apply in-depth knowledge of the Care Act 2014, including Section 22 and the limits of local authority provision. Explore and support appropriate joint funding arrangements where applicable.
