ChristianaCare is seeking a full-time Clinical Pharmacy Specialist to support and expand pharmacy services within the Center for Weight Management. This is a newly approved position designed to enhance patient access, improve medication stewardship for anti-obesity medications, reduce provider burden, and support system-wide cost savings initiatives. The Clinical Pharmacy Specialist will be embedded within the Center for Weight Management and function as an integral member of the interdisciplinary care team, providing comprehensive medication management for patients receiving pharmacologic therapy for obesity and related cardiometabolic conditions. Principal Duties and Responsibilities Provide advanced direct patient care through comprehensive medication management services for patients with complex and high‑risk conditions, utilizing the JCPP Pharmacists' Patient Care Process, including assessment, advanced pharmacotherapy decision‑making, and longitudinal outcome monitoring Independently manage and optimize complex medication regimens within defined specialty areas, including modifying therapy; ordering, interpreting, and acting on laboratory results; and providing advanced therapeutic monitoring in accordance with collaborative practice agreements, privileging, policies, protocols, state scope of practice, and organizational standards Serve as a clinical leader and subject‑matter expert within interdisciplinary care teams by providing consultation and guidance on complex pharmacotherapy, clinical pathways, and specialty‑specific best practices Develop, implement, and oversee advanced, individualized care plans addressing adherence, safety, therapeutic effectiveness, and patient‑reported outcomes, with accountability for high‑level clinical decision‑making Lead or significantly contribute to clinical program development, protocol optimization, population health initiatives, and innovative care models within assigned specialty areas Deliver advanced patient education and self‑management support for complex disease states, high‑risk therapies, and specialty populations, emphasizing safety, care transitions, and long‑term outcomes Provide clinical oversight and leadership related to prior authorization strategy, benefits navigation, utilization management, and adherence interventions, ensuring alignment with evidence‑based, patient‑centered care Serve as a primary preceptor, mentor, and educator for pharmacy residents, students, and pharmacists, supporting advanced clinical training, competency development, and practice advancement Lead or actively participate in quality assurance, performance improvement, research, and outcomes reporting initiatives, including analysis of quality metrics, identification of system‑level improvement opportunities, and development of corrective action plans Maintain exemplary clinical documentation to support continuity of care, billing integrity, regulatory compliance, and accreditation readiness Contribute to organizational compliance with regulatory and accreditation standards through leadership in patient safety, clinical quality, staff competency development, and continuous survey readiness Represent the Department of Pharmacy Services on interdisciplinary, service‑line, and organizational committees as a clinical expert May require intermittent local or regional travel to support specialty clinics, meetings, or multi‑site responsibilities Education and Experience Requirements Doctor of Pharmacy (PharmD) from an ACPE‑accredited school of pharmacy required ASHP‑accredited PGY1 Pharmacy Residency required OR a minimum of 3 years of relevant ambulatory clinical pharmacy experience ASHP‑accredited PGY2 Pharmacy Residency required OR a minimum of 3 additional years of advanced ambulatory clinical pharmacy experience beyond PGY1 residency or PGY1‑equivalent experience Active, unrestricted Pharmacist licensure in the State of Delaware required prior to hire Active, unrestricted pharmacist licensure (or eligibility for licensure by reciprocity) in additional states where patient care is provided, as applicable, required or obtained within 2 years of hire Board certification required or obtained within 2 years of hire Why ChristianaCare?
