Liquid Personnel is seeking an experienced Occupational Therapist to support one of its clients in meeting increased demand following the successful rollout of their Home First contract. This is a high impact reablement role, focused on enabling individuals to maintain independence and reducing the need for long-term care support. What will your responsibilities be? Complete strengths-based assessments in line with Care Act principles Develop reablement-focused care plans to maximise independence Provide expert advice on equipment and adaptations Manage a diverse community caseload, including home visits and hospital settings Work closely with multi-disciplinary teams to support safe discharges and prevent admissions Maintain accurate records and contribute to service outcomes
