- Duties and Responsibilities
- Person-Centered Planning Leadership
- Lead the development, implementation, and ongoing review of individualized support plans
- Oversee person-centered planning processes to ensure meaningful participation and measurable outcomes
- Review and approve support plans to ensure alignment with best practices and individual goals
- Collaborate with Program Managers, families, and support networks to enhance inclusion, independence, and community participation
- Develop and maintain planning resources to ensure consistency and effective individualized planning across programs
- Specialized Supports & Behavioural Oversight
- Lead identification and response to complex behavioural and specialized support needs
- Review and support implementation of behavioural and specialized strategies in collaboration with SSRS and external professionals
- Establish and maintain relationships with healthcare and community professionals to support coordinated care
- Monitor outcomes, documentation, and follow-up of support strategies
- Support staff in implementing strategies safely and effectively
- Lead rights review processes and support strategies to reduce rights restrictions

Job Purpose The Senior Manager, Person-Centered Supports & Specialized Services is responsible for leading the design, implementation, and continuous improvement of individualized, person-centered supports across DASCH programs, with a strong focus on inclusion, independence, and quality of life. This role integrates specialized supports, behavioural and clinical-informed practices, financial planning, and life skills development with strategic and operational oversight to ensure the wellbeing, safety, and long-term success of the people DASCH supports. The position provides leadership in individualized planning, workforce competency development, and program evaluation while ensuring services are responsive, inclusive, evidence-informed, and aligned with organizational standards. This role works collaboratively across Programming, SSRS, Learning & Development, and Human Resources to strengthen service delivery, build staff capacity, and drive measurable improvements in outcomes for the people DASCH supports.
