On a daily basis you will: Provide direct client care and care coordination ensuring that services are provided in accordance with the Rehabilitation at Home model of care Manage the program's intake function including cohort allocation and oversight Work collaboratively with a wide range of stakeholders including St Vincent's Care at Home colleagues and external service providers Contribute to the development and delivery of virtual care solutions nationally Drive improvement initiatives and service development activities to support the growth of the program nationally Your Contribution Relevant clinical experience in acute and/or community setting Current nursing or allied health registration with AHPRA Collaborative team work and a commitment to quality and continuous improvement initiatives is integral to the role
