Pay Range: $18.57 - $23.23 Scheduled Weekly Hours: 40 Position Overview Performs a wide range of clerical duties and a variety of office procedures in support of communication and coordination among members of the Capital Health PACE clinic. Serves as primary point of contact between participants and caregivers to clinic team, and provides clerical support through answering phones, greeting participants, and assisting with coordination of care for the participants. Handles inter and intra departmental communications to assist with the coordination of participant care while supporting the clinic and staff. Ensures that participants are provided with and receive needed durable medical equipment and repairs as needed. Reviews outside physicians' consults, labs, and other medical records and assures that the orders sent to the appropriate clinic staff for follow up before getting scanned into the chart. Ensures accurate and timely scanning of medical record documents, labs, and other documents. Assists in maintaining clinic flow for both participants and staff. ESSENTIAL FUNCTIONS Orders all oxygen, respiratory supplies, and equipment in required systems. Interacts with outside agencies as required. Keeps an updated list of all participants with orders for oxygen and follows up regularly with these participants to make sure that they have an adequate supply of back-up oxygen tanks in the home. Monitors medical supply levels in the clinic and order as needed. Orders medical supplies for participants through system and places new orders. Orders wound supplies for participants when needed. Ensures that there is an order in place prior to ordering. Enters all authorizations in Electronic Medical Record (EMR) as needed or requested. Acts as the liaison for optometry visits. Assists with the order and procurement in EMR and follows up in a reasonable amount of time to determine eyeglasses availability. Notifies the participant and arranges pick-up and delivery. Ensures receipt. Contacts all relevant parties to schedule all stat testing ordered by physician. Ensures that the participant is pre-registered, and interacts with employees in transportation for the participant to be taken to appointments. Ensures that all paperwork, including face sheet, consultation sheet, and orders, are sent with the participant. Registers participant in hospital system for all completed lab work prior to lab work being sent to hospital. Scans medical record documents into the EMR. Stamps scanned documents with date received. Updates the daily wrap-up report throughout the day and sends to required staff at the close of the workday. Collects and scans into the EMR all outside appointment consultation reports and labs. Ensures that these records are scanned to the physician after each visit. Ensures that all labs received are forwarded to the appropriate clinic RN for further evaluation. Follows up on outstanding labs, consults, and test studies to ensure that the proper documentation has been received in a timely manner. Documents every follow up attempt. Notifies physician with anything outstanding due to unsuccessful attempts. Triages incoming clinic phone calls to RNs and physicians as appropriate. Helps manage the day-to-day clinic operations including assisting to meet participant requests, obtaining meals for participants and transporting throughout the center as needed. Cross-trains for other positions and covers these roles as needed. Establishes and maintains open lines of communication with the nurses and staff. Communicates any problems, concerns, or issues promptly to the director. Places orders in DME tracker for supplies and equipment, and orders for other departments as needed or requested.
