Day Shift - 7.5 Hours (United States of America) Core Responsibilities - Identify, access, and refer patients and families to appropriate community and support services - Provide care navigation and logistical support following emergency room or inpatient discharge - Develop and maintain relationships with primary care and specialty providers to ensure coordinated care - Participate in interdisciplinary care team meetings and collaborate across departments - Conduct outreach in the emergency room and outpatient clinic, particularly for asthma-related cases - Maintain accurate documentation as required by the Department of Pediatrics, PCMH, DSRIP, and the Asthma Coalition of Staten Island - Assist with data collection and entry for program tracking and reporting - Attend community-based meetings and represent RUMC at outreach events several times per month - Support general and condition-specific initiatives related to asthma, obesity, mental health, maternal depression, and developmental delays Core Competencies - Knowledge of pediatric care systems and community health resources - Strong communication and organizational skills - Ability to work collaboratively across departments and with external partners - Culturally sensitive and family-centered approach to care Customer Service Competencies - Maintains professionalism, compassion, and respect in all interactions - Communicates clearly and responsively with families and team members - Ensures confidentiality and a supportive experience for all patients - Responds appropriately to concerns and feedback
