Job Description Summary Occupational Therapist provides skilled Occupational Therapy service on an intermittent basis to clients in their homes, in accordance with the physician's orders How will you make an impact & Requirements Assists the physician in eva l uating the c lient ' s level of function by apply i ng diagnostic and prognostic procedures. Conducts initial and ongoing comprehensive assessments of the client's needs including Out c o m e and Assess m ent Info r m ation Set (OAS I S) assess m ents. Evaluates the client ' s significant other(s) and the home situat i on to determine the instructions that will be required; the type of family assistance that will be available in caring for the client; and, other Agency and community services that will be required. Ensures that the physicia n ' s orders a r e app r opriate and disc u sses necessary changes. Assists in d e velop m ent and i m ple m entation of the interdisciplinary client care plan to include O c cupational Ther a py i n con s ultation with the physician. Guides and instructs the client in prescribed therapeutic self-care methods as well as creative activities that are directed toward improving indep e ndence and physical and m ental functioning. Instructs client in care and use of wheelch a irs, braces, splints, and p r osthetic and orthotic devices. Teache s , supervises and counsels the family and client in the total Occupational Therapy program and any related probl e m s that the client may have at ho m e. Establishes goals based on t h e client ' s c u rrent level of functioning and potential for i m prove m ent; per f or m s r e -evaluations as indicated. M a kes arrange m ents for outp a tient services w h en procedures that cannot be given in the home are required. Evaluates the effectiveness of occupational therapy intervent i on and updates the client care plan as needed. Prepares clinical/progress notes on the day of the visit and enters note into the EMR within required timeframe (24 hrs. for non-Oasis visits and 48 hrs. for Oasis visits). Co mmu nicates with the physician on a r e gular basis regarding c lient ' s status and obtains additional orders, if necessary. Co mmu nicates with o t her team m e m b ers and, w h en app r opriat e , instructs them in Occupational Therapy techniques that they may use while working with the client. Superv i s es the ho m e health aide eve r y two we e k s wh e n Occupational Therapy is the on l y p r ofessional discipline invo l ved in t h e client ' s care. Superv i s es the Occupational The r apy Assistant at least once m onth l y. Participates in Agency activities, in-services and meetings, when appropriate. Confir ms , on a weekly basis, the scheduling of visits with the Director of Nursing in order to c o ord i nate A g en c y visits. Participates with the st a ff, the client, and the physician in discharge activities and completes an Occupational Therapy Discharge S u mm ary. Assumes responsibility for s e lf-development by continually striving to imp r ove his/her health care knowledge through education a l progra m s, attendance at wo r k shops and conferences, active participation in p r ofessional and related org a nizations and i nd i vidual research and reading. Is knowledgeable of the Agency's organization, functions, and services. Complies with Agency dress code policy. Maintains efficient flow of Agency work by reporting to work at assigned or scheduled times; follows procedures for notification of absence or time off requests. Maintains current TB testing (refer to Health Screening Policy).
