Liquid Personnel is recruiting an Adult Social Worker to join its client's Older People & Hospital Team, based in Conwy. What will your responsibilities be? In this role, you will; Hospital Discharge Planning - Lead timely, safe, and person‑centred hospital discharge planning under the Care Act and Discharge to Assess pathways, ensuring assessments are proportionate, strengths‑based, and coordinated with health partners. Complex Assessment & Safeguarding - Complete holistic assessments, Mental Capacity Act decisions, and safeguarding enquiries for older adults with complex medical, cognitive, or social needs, ensuring robust risk management and legal compliance. Multi‑Agency Coordination - Work collaboratively with ward teams, community health, therapy services, providers, and families to develop effective support plans, prevent delays, and promote independence and wellbeing post‑discharge. Qualifications & Experience: To be successful in this role you must have: Social Work England registration Eligible to work in the UK Hold a full UK driving licence Post-qualified experience Degree level or equivalent in Social Work
