Behavioral Health Clinician (Family Health Center)

About the role

Responsible for the clinical assessment and outpatient treatment for patients seeking mental health treatment. Serves as a member of an interdisciplinary treatment team. Education Five to six years of formal training or education beyond the high school level (e.g., Masters Degree or five-year program in a mental health discipline). Certification, Registration & Licensure Licensure required to obtain: Must be license eligible. Licensed by the state of New Hampshire to perform mental health practice within two years of hire date.

Responsibilities

• Conducts bio-psycho-social mental health assessments of patients and develops patient-centered treatment plans.
• Administers direct patient care.
• Participates in education and training of Behavioral Health Interns and Family Medicine Residents.
• Documents care provided according to Medicare and other compliance standards, and CHMG and practice standards.
• Monitors schedule in order to maximize patient access.

Requirements

• At least two years practicing mental health counseling preferred.
