About the role

The Per Diem Physical Therapist provides skilled hospice care services to adult and geriatric patients across a variety of settings, including private homes, assisted living communities, and inpatient facilities.   • Excellent interpersonal and communication skills.

  • Valid driver's license and reliable transportation required.
