About the role
This role is employed by Phoenix Med Health (MSO) , a healthcare management organization supporting medical practices across operations, growth, and technology. Responsibilities
- Own and grow relationships across facilities, home health agencies, hospice, and other referral sources
- Represent Active Vital Care in the field as the primary point of contact
- Drive consistent patient referrals and new patient registrations
- Conduct regular in-person visits to partners
- Identify and onboard new referral partners
- Track outreach, referrals, and pipeline activity in CRM
- Coordinate internally with scheduling, clinical, and operations teams
- Resolve issues quickly and maintain strong partner satisfaction
- Continuously increase referral volume per account
