Responsibilities
- Provide compassionate, hands-on care that supports patient comfort, safety, and independence:
- Assist patients with daily living activities, including bathing, dressing, grooming, and toileting
- Support mobility, transfers, and basic exercises in alignment with the care plan
- Help maintain a clean, safe, and comfortable home environment
- Prepare meals and assist with feeding as needed
- Take and record vital signs when required and trained to do so
- Assist with medication self-administration as directed
- Observe and report changes in patient condition to the clinical team
- Document care provided accurately and timely within the EMR system
- Participate in team meetings, trainings, and care coordination activities
- Provide compassionate support to patients and families during each visit
Requirements
- We're seeking individuals who are dependable, compassionate, and comfortable working independently in a home-based setting:
- Meets state and federal requirements for Home Health Aide certification or training
- At least 18 years of age
- Ability to read, follow care plans, and document services provided
- Current CPR certification
- Valid driver's license, reliable transportation, and auto insurance
- Ability to work independently with strong time management skills
- Demonstrates empathy, patience, and professionalism in all interactions
