Responsibilities

- Serves as a community resource by supporting outreach and engagement efforts, providing education on available services, and fostering connections between patients, caregivers, and community-based resources to enhance access to care and promote overall health and well-being.
Requirements
- Education
- Bachelors Degree, Social Work, Gerontology, or a health-related field (Required) or
- Masters Degree, Social Work, Gerontology, or a health-related field (Preferred)
- 5-6 years Demonstrated experience supporting individuals with memory loss and working collaboratively with their families. (Required)
- Licenses and Certifications
- CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)
- Certified Dementia Practitioner (CDP) - National Council of Certified Dementia Practitioners (NCCDP) (Preferred)
- PAC Certified Dementia Champion (PAC-DC/L) (Preferred)
- Drives personal vehicle for RHS business 25% or more of the time to perform essential functions of the job
- Valid Drivers License Required

Williamsburg, Virginia Hiring Range $68,000.00 - $93,500.00/Annual Actual pay is determined based on job-related factors such as relevant experience, education, credentials, skills, internal equity, and business needs.
