Responsibilities

Our experienced and dedicated team assist the resident's transition into a long-term care facility and continue to work with them on any challenges they may be facing, such as anxiety, depression, and other mental health issues. Our Licensed Social Workers and Counselors deliver personalized services within the nursing home and/or assisted living facility, ensuring that each client's unique mental health needs are met with compassion and expertise.
