If you're looking for a flexible schedule, meaningful patient relationships, and the opportunity to make a lasting impact every day, we'd love to meet you. ๐ You Might Be a Great Fit If You: Conduct comprehensive speech-language evaluations to assess communication, cognitive, and swallowing needs; develop individualized treatment plans, provide evidence-based interventions, and monitor patient progress. Deliver skilled speech therapy services in accordance with physician orders and the CMS Conditions of Participation (CoPs) . Maintain timely, accurate documentation while ensuring compliance with CMS, payer, and agency standards. Collaborate with patients, caregivers, physicians, and the interdisciplinary team to optimize communication, cognition, swallowing, safety, and functional independence. Educate patients and caregivers on communication strategies, swallowing precautions, cognitive techniques, adaptive tools, and individualized Home Exercise Programs (HEPs) to support functional recovery. Reassess patient progress, modify treatment plans as appropriate, and communicate changes to the care team. ๐ผ What We're Looking For: Degree from an accredited Speech-Language Pathology program. Current Michigan Speech-Language Pathologist (SLP) license in good standing. Home health experience preferred (minimum one year). Strong clinical judgment, critical thinking, and problem-solving skills. Excellent communication, interpersonal, and patient education skills. Ability to work independently while collaborating effectively with an interdisciplinary team.
