Position Summary The Contingency Management (CM) Case Manager provides comprehensive coverage to clients participating in GLIDE's Contingency Management program, which is an evidence-based treatment that uses incentives to encourage positive behavioral changes related to substance use. GLIDE's CM Program will focus on clients who use stimulants and are interested in managing and/or changing their relationship to stimulant use. This program will offer three groups weekly, individual check-in's, drop-in hours available for clients and clients will receive financial incentives for UAs negative for stimulant use as well as other psychosocial goals as defined by the client in collaboration with GLIDE. Through outreach, health education, counseling, and case management, this position will support a client case load of 20 to 30 clients to maintain program adherence and achieve progressive, positive health and wellness outcomes. The Case Manager should understand the social determinants of health and will support assistance with housing, benefits, critical identification documentation obtainment, health care engagement, access to behavioral health supports, and any other needs identified by the client, and any other. This position involves direct in-person client contact with diverse, high-risk individuals, many of whom are active substance users, substantial community outreach, and group facilitation.
