Provide direct supportive services to individuals with mental health symptoms Assist clients in generating proactive solutions to barriers regarding their recovery Complete documentation in a timely and accurate manner Complete trainings and attend meetings as directed Participate in care coordination to collaborate with community partners, caregivers, family members, physicians and other treatment providers regularly Act in accordance with the established mission, vision, and values. Abide by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI). Maintain effective communication and professional working relationships with patients/clients and their representatives, team members, contractors, physicians, peers, outside agencies, and the public.
