Job Title: Social Worker - Preparing For Adulthood Team Location: Bromley Pay Rate: £37 Liquid Personnel is currently recruiting for an Adult Social Worker for the Preparing For Adulthood Team based in Bromley. What will your responsibilities be? Provide an assessment service to individuals, their carers and other agencies in accordance with Care Act (2014) criteria. Initiate and co-ordinate care and support plans for and with individual service users and support plans for carers, in conjunction with professional colleagues and representatives of voluntary and private agencies.
