Liquid Personnel is seeking an Adult Social Worker to deliver high-quality, person-centred social work within a multi-disciplinary rehabilitation team based in Bromley. The role involves supporting adults with neurological conditions and those transitioning from hospital settings under the Discharge to Assess (D2A) model. It focuses on timely assessments, care planning, and promoting recovery, independence, and safe discharge. What will your responsibilities be? Undertake Care Act 2014 assessments and develop strengths-based support plans for individuals with neuro-rehabilitation needs or post-hospital discharge. Work collaboratively with occupational therapists, physiotherapists, nurses, and hospital discharge teams to ensure safe and effective transitions from acute settings. Complete Mental Capacity Assessments, Best Interest decisions, and contribute to DoLS processes where required. Support individuals with acquired brain injuries, stroke, MS, and other neurological conditions through rehabilitation and community reintegration. Respond to safeguarding concerns and contribute to Section 42 enquiries. Monitor and review care packages to ensure they meet evolving needs and promote independence. Maintain accurate and timely case records using the council's case management system (e.g., Liquid Logic, Mosaic). Advocate for service users and their families, ensuring their voices are central to decision-making. Contribute to service development and continuous improvement within the rehab and D2A pathways.
