Your opportunity Full time Position based at Castle Hill As a Care Partner for our Seniors Communities, you'll walk alongside seniors and their families, helping them navigate each stage of their aged care journey. From assessing needs to coordinating services, you'll make sure every client receives the right support, tailored to their goals and funded care options. In the role you will: Support seniors with care planning, assessments, and coordination to help them live independently. Conduct assessments and create initial care plans and budgets in collaboration with clients Build strong relationships with clients and families, tailoring services to individual needs. Advocate for clients as their circumstances change and ensure they receive the right support.
