Develop and implement person-centred nursing care plans based on residents' needs and preferences. Administer medications in accordance with organizational guidelines and protocols. Perform nursing interventions and monitor residents' health conditions. Escalate changes or abnormalities in residents' health conditions when necessary. Maintain accurate and timely nursing documentation. Ensure a safe and conducive environment for residents. Facilitate Advance Care Planning discussions. Educate residents, families, and caregivers on health management and self-care. Conduct health promotion and disease prevention activities. Coordinate care with multidisciplinary teams and external healthcare providers. Support care referrals and transitions between different care settings. Participate in quality audits and quality improvement initiatives. Identify and report risks and safety concerns in accordance with organizational policies. Practice infection prevention and control measures. Execute emergency protocols during public health threats or emergencies.
