The primary duties include developing and managing the program work plans, building partnerships, and implementing and evaluating program strategies. Facilitates group or coalition meetings and creates and delivers on grant deliverables and program content. Collaborates with Community Health & Education leadership in strategy development and Implementation planning. Monitors state and national models of partnership across business, community and healthcare sectors, offering guidance where there are opportunities to align with best practices. Engages with schools and community partners, seeking out strategic opportunities for collaboration and alignment with grant/contract and CW priorities. Essential Functions - Leads, coordinates and implements assigned program initiatives in collaboration with internal and external stakeholders. - Develops strategic plan for implementing, monitoring, training and evaluating program. - Manages the financial components of the program, including grant/contract/financial spreadsheet management and associated deliverables. Leads the development of MOUs or MOAs with partnering agencies. In partnership with leaders and Foundation, assists with finding and securing grant and philanthropic opportunities. - Identifies, creates and executes processes necessary to meet specific program objectives and compliance, standards and requirements, including managing timetables, work plans, budget spreadsheet management and reports. - Creates and disseminates educational materials, programming and materials within CW and in the community to enhance program education and awareness to patient families, employees and other community members. - Collaborates with state, local and national partners, grant funders, the CW Foundation, program staff and other organizations to identify and carry out program strategies. Facilitates meetings among partners and prepares informative and well-organized presentations as needed. - Serves as an on-going resource/expert for internal and external stakeholders, partners and agencies, to include media opportunities. Provides content for newsletters, website, and social media platforms (ex: Facebook, Twitter and Blogs). - Develops training modules provides training and support to program partners. - Establishes measurable goals and objectives for the program in accordance with the program's design and implementation plan. Manages data collection, dashboard development and maintenance. Education: - Bachelor's Degree in education, human services, health care management or related field required - Master's Degree related field preferred
