Job Title: Social Worker - Adult Learning Disability - Bolton Location: Bolton Pay Rate: £30 Liquid Personnel is currently recruiting for an Adult Social Worker to join a Learning Disability based in Bolton. What will your responsibilities be? Conduct Care Act assessments, strengths-based support planning, and carers assessments. Complete capacity assessments, Best Interest decisions, and Continuing Healthcare assessments. Work collaboratively with multidisciplinary teams and lead safeguarding investigations. Support individuals in transitioning to alternative accommodation, including resettlement from out-of-area placements. Prepare reports for tribunals, hospital discharges, Community Treatment Reviews, and Court of Protection applications.
