Job Title: Hospital Social Worker Location: Warwickshire Rate: Up to £34 per hour Our client in Warwickshire is seeking an experienced Hospital Social Worker to join their team. What will your responsibilities be? As a Hospital Social Worker, you will conduct thorough social care assessments to determine the needs of patients preparing for discharge. You will develop and implement tailored care plans and collaborate with medical, nursing, therapy, and discharge teams to ensure a smooth and timely transition from hospital to home or care settings. Managing a caseload effectively, you will also undertake additional assessments such as Mental Capacity Assessments, Safeguarding enquiries, and Continuing Health Care checklists. This role operates within a hybrid working model, requiring flexibility between home-based and hospital site work.
