Job Description: The Palliative Medicine Service Clinical Social Worker reports to the Palliative Medicine Service Manager and works collaboratively with the Interdisciplinary Palliative Team as well as patient's larger health care team. Caseload can consist of inpatients, outpatients, and their families. The Palliative Social Worker is responsible for completing thorough psychosocial assessments with patients with serious illnesses and their family members. Based on assessment, the Palliative Social Worker develops treatment plans, provides supportive counseling, crisis intervention, and makes referrals to appropriate community resources . Social work services include assisting patients and their families in understanding and coping with the emotional and social problems related to their health status.
