** Internal Workers - Please log into your Workday account to apply ** Huntington Hospital Employee Login Expectations: Responsible for supervising the daily operations and workflows of the Discharge Care Center (DCC) and transitional care services across the continuum of care. Oversees staff operations, workflow coordination, patient transition activities, and departmental processes that support continuity of care, patient engagement, readmission reduction, and post-discharge outcomes. Partners with interdisciplinary teams to support an integrated care coordination model spanning inpatient, ambulatory, post-acute, and community settings. Provides operational oversight, staff development, quality monitoring, and process improvement support in collaboration with the Executive Director of Case Management. EDUCATION: Bachelor's degree in nursing (BSN) from an accredited nursing program is required. EXPERIENCE/TRAINING: Minimum of three (3) years of clinical recent clinical experience in hospital case management, care coordination, transitional care, and discharge planning required. Previous Lead, Charge Nurse, Preceptor, or supervisory experience preferred. LICENSES/CERTIFICATIONS: Required: Current unrestricted Registered Nurse (RN) license issued by California Board of Registered Nursing (BRN) required. Preferred: Case Management certification (ACM, CCM, or equivalent) preferred.
