Responsibilities
- Deliver Compassionate Hospice Care
- Provide direct patient care in accordance with the Nurse Practice Act, with a focus on end-of-life support
- Implement individualized plans of care under the supervision of a Registered Nurse
- Deliver effective pain and symptom management, monitoring and adjusting based on patient response
- Support patients with daily care needs while ensuring comfort, dignity, and safety
- Educate patients and families on disease progression, medications, and comfort measures
- Coordinate & Communicate
- Document care accurately and timely in the EMR
- Communicate changes in patient condition to the RN and care team
- Collaborate with interdisciplinary partners (RN, HHA, Social Work, Chaplain, Therapy)
- Participate in care planning and coordination to ensure holistic support
- Commit to Excellence
- Follow best practices and clinical standards in hospice care
- Maintain proper use of equipment and adhere to infection control protocols
- Participate in quality improvement initiatives and team development
- Continue professional growth through training and education
- What You Bring
- Active LPN/LVN license in the state of practice
- Current CPR certification
- 1-3+ years of nursing experience preferred (hospice, home health, or med-surg)
- Strong clinical judgment, communication skills, and emotional intelligence
- A genuine passion for patient-centered, end-of-life care
- Valid driver's license, reliable transportation, and auto insurance
