• HCPC registration.
• Bachelor's degree in Physiotherapy or a related field.
• Experience in community or discharge planning settings is desirable.
• Strong clinical reasoning and decision-making skills.
• Excellent communication and interpersonal abilities.

You will work collaboratively within a multidisciplinary team and your caseload will consist of supporting triage referrals for both the Discharge Hub and Home First services.
