Treatment will be provided in a remote-based, telehealth format. The behavioral health clinician will provide short-term psychotherapy across the continuum of care, including identifying eligible patients based on defined criteria, providing psychosocial assessment, developing and executing treatment in alliance with patient's goals, and ultimately ending care with a firm discharge plan in place. This role will additionally be responsible for providing crisis support to clinic patients identified as needing urgent behavioral health needs and ensuring they are provided with and/or connected to the next appropriate level of care. The behavioral health clinician will work in collaboration with the behavioral health team as well as the primary care clinic's multidisciplinary team including the patient's primary care provider and clinic leadership. This is an open-rank faculty recruitment at either the Instructor, Senior Instructor, or Assistant Professor level. Applicants must be legally authorized to work in the United States without requiring sponsorship. Key Responsibilities : Work collaboratively with practice stakeholders to deliver integrated behavioral health services in a telehealth modality. Provide short-term behavioral health treatment to targeted groups of patients; developing comprehensive initial assessments, treatment and discharge plans. Refer patients to higher level of care (e.g. specialty treatment) -or community long term care - as appropriate. Coordinate and revise treatment plan with input from interdisciplinary team and primary care provider. Provide routine patient progress monitoring. Support the management of patient referrals, including coordinating care for those patients not meeting criteria for clinic based behavioral health services. Support clinic in developing community and campus referral sources as needed. Work collaboratively with clinic staff to ensure efficient scheduling and placement of referrals. Work Location: Remote - This role is eligible to work remotely, but the employee must be in the United States.
