An opportunity has arisen for an Occupational Therapist to join our client at the Richmond and Wandsworth Reablement Service, supporting adults (18+) to regain independence, improve wellbeing, and achieve meaningful outcomes. You will manage a caseload requiring assessment, review, and ongoing support, completing weekly home visits and working closely with residents, families, carers, care agencies, social work teams, and hospitals.
What will your responsibilities be?
Deliver reablement‑focused OT assessments and interventions
Complete Care Act assessments, Mental Capacity Assessments, carers' assessments and reablement reviews
Undertake Moving & Handling risk assessments and create Personal Handling Plans
Prescribe and recommend equipment and minor adaptations
Maintain accurate case records on Mosaic (daily updates required)
Work in line with Care Act 2014 and local authority statutory duties
