About the role

You're the kind of engineer who gets curious when things don't behave as expected. When audio drops, latency spikes, devices behave inconsistently, or streams become unreliable, you want to understand why. You've spent time building systems where audio and real-time media matter. Maybe you've worked on streaming platforms, communication products, podcasting infrastructure, video conferencing, live broadcasting, or other media-heavy applications. You've seen the challenges that come with real-time systems and know that delivering a seamless experience requires careful engineering across the entire stack. At Tandem Health, you'll help build and scale the audio infrastructure powering millions of healthcare conversations. From device capture and streaming to transcription and AI-powered documentation, you'll work on systems that need to be reliable, low-latency, and production-ready at scale. This is a highly impactful role where you'll work closely with product, AI, and platform teams to ensure that every conversation reaches our systems with the quality and reliability needed to support clinicians across Europe.

Responsibilities

  • Build and improve the systems that power real-time audio streaming throughout our platform.
  • Work across the full media pipeline, including device capture, buffering, transport, streaming, transcription, monitoring, and quality assurance.
  • Design and operate reliable, scalable infrastructure capable of handling large volumes of real-time audio data.
  • Investigate and solve challenging production issues related to latency, connectivity, audio quality, and device compatibility.
  • Collaborate with engineers across frontend, backend, infrastructure, and AI teams to build seamless end-to-end experiences.
  • Improve observability, testing, and tooling to ensure our media systems remain stable as we continue to scale.
  • Help shape technical direction and engineering best practices for real-time media systems at Tandem.
  • What You Bring
  • Experience building and operating production systems involving audio, video, streaming, or other real-time data.
  • Strong software engineering fundamentals and experience building reliable backend systems.
  • Familiarity with technologies and protocols commonly used in real-time media systems such as WebRTC, RTP, RTCP, WebSockets, streaming infrastructure, or similar technologies.
  • Experience troubleshooting performance, latency, reliability, and scalability issues in production environments.
  • A systems-oriented mindset and a strong desire to understand how things work beneath the surface.
  • Ability to work across multiple layers of the stack and collaborate effectively with engineers from different disciplines.
  • Strong communication skills and a pragmatic approach to solving complex technical problems.
  • Bonus points:
  • Experience working with large-scale audio processing pipelines.
  • Experience with speech-to-text, transcription, or conversational AI systems.
  • Experience building products where audio or video is a core part of the user experience.
  • Experience with frontend development in React and TypeScript.
  • Experience with C# and Windows-based development.
  • Experience working in healthcare, regulated environments, or other reliability-critical domains.
