Responsibilities
- Coordinates and plans physician requested multidisciplinary conferences; tracks scheduled procedures and follows pathology results, coordinates with pathologist and radiology the collection of slides and films related to a diagnosis and monitors outcomes of meetings.

- Identifies, recommends and continues to establish supportive services to meet patients' needs.
- Develops programs and provides staff and community education as required; maintains reference library.
- Pursue opportunities for public speaking to promote cancer care services at Northwestern Medicine Lake Forest Hospital.
- Required :
- Registered Nursing license issued by the State of Illinois.
- Bachelors of Science Nursing.
- BLS through American Heart Association.
- Must demonstrate broad and thorough knowledge of the theories, principles and practices of oncology care.
- Five years of oncology related work experience.
- Must be a self -directed independent practitioner.
- Preferred :
- Masters of Science Nursing

Additional Information
**This is a Full Time position at 40 hours per week located at Lake Forest Hospital. The Nurse Navigator will support our Hem Onc Inpatient team with care coordination on the unit and at time of discharge.
