Accept and manage patients within your designated geographical area of coverage Plan, prepare, and implement customized therapy programs designed to restore overall function and promote independence in the home setting Develop and maintain individualized care plans/goals in collaboration with physicians, patients, and their families Ensure timely and accurate documentation of all patient visits and therapy services Effectively communicate discharge planning with patients, families, and interdisciplinary team members Maintain compliance with all clinical and documentation standards Physical Therapist Program Graduate. TN Physical Therapist License or eligible. Liability insurance Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
