JOB RESPONSIBILITIES Patient Information - Collects medication history information from patients or caregivers to be used in medication reconciliation. Medication Orders - Performs unit dose preparation for cartfill and first doses. Non-sterile Non-hazard dose preparation for first fill or cartfill including preparation of oral syringes from liquid dosage forms. Delivery of patient specific doses. Prepare and perform ADC replenishment. Performs outpatient phone triage. Performs insurance adjudication. Performs medication storage inspections. Program Management - Manages medication programs (i.e. Synagis program) Medication Preparation - Prepares sterile hazardous medication (i.e. IV Chemotherapy). Compounding - Performs Sterile compounding or non-sterile hazardous compounding. Under authorized program is able to check the work of another pharmacy technician. Manages medication doses on the nursing unit. Performs inpatient phone triage. JOB QUALIFICATIONS High school diploma or equivalent 3+ years of work experience in a related job discipline Must be PTCB certified and OH Board of Pharmacy certified Primary Location Lincoln Building Schedule Full time Shift Day (United States of America) Department Home Care Pharmacy Employee Status Regular FTE 1 Weekly Hours 40
