The Mobile Integrated Health (MIH) Paramedic delivers high-quality, patient-centered medical care in the home setting as part of our Mobile Integrated Healthcare (MIH) program. This role extends clinical services beyond traditional emergency response by providing scheduled, proactive, and post-discharge care to patients in collaboration with healthcare providers. The MIH Paramedic works closely with healthcare organizations, physician groups and accountable care organizations (ACOs) to reduce unnecessary emergency department utilization, prevent hospital readmissions, and improve overall patient outcomes. This position requires strong clinical judgment, excellent communication skills, and a passion for delivering compassionate care directly to patients in their homes. Requirements

• Current Illinois Department of Public Health (IDPH) Paramedic License.
• Minimum two (2) years of experience working as a paramedic in an EMS or healthcare setting.
• Demonstrated knowledge of clinical protocols, emergency medical procedures, and patient care standards.
• Valid driver's license and clean driving record.
• Position requires working vehicle in good condition.
• Strong communication, interpersonal, and problem-solving skills.
• Ability to work independently and collaboratively within a multidisciplinary team.
• Commitment to patient-centered care, cultural competence, and ethical conduct.
• Must pass a background check.
• Must possess a valid CPR card.
• Must complete CDH EMS System entry process.
• Stay current with EMS System requirements.
• Proficient in Epic & Image Trend (preferred).
• Must possess uncompromised character, high ethical standards, scholarly concern for improvement, and motivation for continued learning.
