Job Description Summary ‎ Physical Therapy Assistant provides skilled Physical Therapy services on an intermittent basis to clients in their homes, under the direction of the Registered Physical Therapist (PT). ‎ How will you make an impact & Requirements ‎ Provides client services that h a ve been del e gated by- and are under the direction of- t h e R e gistered Phy s ical Therapist (PT) Assists the PT in evaluation and develop m ent of a rehabilitative Plan of Care and in periodic re-evaluation as required. Participates in the preparation of clinical and daily progress no t es which are reviewed by the PT and enters in EMR within 24 hours. Perfo r m s routine strength e ning and therapeutic exercises and assis t s the patient with the use of self-help devices, as directed. Assists the PT in providing ins t ructions and in demonstrating exercise progra m s to the patient and fa m ily. Identifies possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contra-indicated m e dication and reports sa m e to the P T. Observes and records activities and findings in the clinical record; informs the PT and the Physician of any reactions to treatment and any changes in the client's condition. Consul t s with PT r egarding the outco m e of ho m e visits and reports physical findings in writing. I n structs other health team personnel includ i ng the ho m e health aides as well as family members, as appropriate, with respect to certain phases of physical therapy, so that they may work with the client. Writes daily p r og r ess notes concerning patients and sub m its same within 72 hours; pr e p a res periodic summary reports as indicated. Attends Rehabilitation Department staff meetings and case conferences. Participates with staff, c l ients, and physicians in discharge activities and assists the PT in c o mpleting the Physical T h erapy Discharge Summary. Confir ms , on a weekly basis, the scheduling of visits with the S upe r v isor / Director of Nursing in order to c o ord i nate A g en c y visits. Assumes responsibility for s e lf-development by continually striving to imp r ove his/her health care knowledge through education a l progra m s, attendance at wo r k shops and conferences, active participation in p r ofessional and related org a nizations and i nd i vidual. Assu m es other duties dee m ed app r opriate a nd necessary by the PT a nd as designated by Registered Nurse Case Managem e nt and t h e Nursing Su pervisor. Is knowledgeable of the Agency's organization, functions, and services. Complies with Agency dress code policy. Maintains efficient flow of Agency work by reporting to work at assigned or scheduled times; follows procedures for notification of absence or time off requests. Maintains current TB testing (refer to Health Screening Policy).
