Job Overview The Case Manager II, Behavioral Health, advances the department's mission by providing experienced referral management and case coordination for patients with behavioral health needs. The Case Manager II, manages and tracks referrals, coordinates communication between providers, and ensures timely linkage to care. The Case Manager II also documents all referral activities, assists with more complex patient navigation, and supports the team in addressing barriers to care. Minimum Requirements Associate's degree in Social Services, Psychology, Human Services, Counseling, or related field required. Bachelor's degree preferred. Minimum of 2 years of experience in healthcare, behavioral health, or social services, assisting patients with case management, referral coordination, and linkage to care, required. Bilingual English/Spanish or English and another predominant language of the population served preferred.
