About the role

Conduct routine and comprehensive health evaluations for patients with chronic illnesses, monitoring key health indicators to proactively manage their conditions. Promote adherence to prescribed medication regimens by educating patients on the significance of compliance, potential side effects, and proper actions to take in case of missed doses Serve as the first point of contact for the designated patient population, providing rapid access and response for urgent concerns and coordinating timely interventions, particularly for high-risk or complex chronic patients. Identify and diagnose new health conditions, including common illnesses and injuries, within the scope of routine primary care. Ensure timely and effective treatment interventions. Collaborate with the Interprofessional Care Team (IPCT) to develop, implement, and adjust individualized care plans based on patient progress and emerging needs. Provide ongoing education and support to patients and their families, empowering them with knowledge and self-management tools for chronic conditions. Coordinate referrals and facilitate communication with specialists, ensuring that their recommendations are integrated into the patient's overall care plan for continuous and comprehensive care. Maintain an up-to-date understanding of available external programs and resources. Appropriately refer patients to these programs to enhance their care and provide additional support beyond the clinic setting. Facilitate smooth transitions for patients moving between different levels of care, such as from hospital to home or from primary care to specialist services, ensuring continuity and comprehensive management of their health and well-being. Work closely with the team to ensure seamless patient care by maintaining accurate documentation of all patient interactions and care plan
